• Critical Situation: 
 
• Call 911 or call the 4s at Intel  
• Stay with the injured person  
• Assign someone on your team to guide the paramedics to the injured worker. (At Intel, identify the building, floor, and column location for the ERT)

• Send a message via text, to FSS PM and FSS Job Superintendent and Owners  
• Contact the GC Safety / Intel Safety, Micron Safety 
• After the worker has been taken for treatment 

• Safe off the area, use danger tape if appropriate to preserve the scene, red tag broken tools and equipment 

• Take pictures of the scene 

• Collect the PTP(s) 

• Interview witnesses 

• Work with GC/Intel to create an incident report  
• Start an FSS Incident Report 
• Interview witnesses 
• Evaluate and record the circumstances of the incident  
• Later, complete the incident report with the help of the injured worker and Safety  
• Assist Safety in filling out the SAIF 801 form or the OCIP where applicable 
 
• Non-Life-Threatening Injury-Requiring Medical Attention: 
 
• Apply first aid as appropriate, or at Intel, report the incident to Safety and arrange to take the worker to the site nurse

• Send a message via text, to FSS PM and FSS Job Superintendent and Owners

• Report the incident to the GC/Intel  
• Assure the worker is picked up and delivered to occupational health or to the Intel site nurse for triage 
If the Safety Team is unable to pick up the worker, take the worker to the urgent care facility. Do not let them drive alone.  
• Begin an incident report 
• Interview witnesses 
• Evaluate and record the circumstances of the incident  
• Later, complete the report with the help of the injured worker and Safety  
• Assist Safety in filling out the SAIF 801 form or OCIP where applicable 
 
First Aid:

• Near Miss:

• Report to the GC or to the Safety leads  
• Begin an Incident Report
