What to do when an incident happens
on your job site
Triage Path
Critical Situation:
• Call 911
• Stay with the injured person
• Assign someone on your team to guide the paramedics to the injured worker. (At Intel, identify the building, floor, and column location for the ERT)

• Send a message via text, to FSS PM and FSS Job Superintendent
• Contact the GC Safety / Intel Safety
• After the worker has been taken for treatment
• Work with GC/Intel to create an incident report
• Start an FSS Incident Report
• Interview witnesses
• Evaluate and record the circumstances of the incident
• Later, complete the incident report with the help of the injured worker and safety
• Assist Safety in filling out the SAIF 801 form or the OCIP form if at Intel
Non-Life-Threatening Injury-Requiring Medical Attention:
• Apply first aid as appropriate, report the incident.

• Report the incident to the GC / Intel
• Assure the worker is picked up and delivered to occupational health or to the Intel site nurse for triage
If the Safety Team is unable to pick the worker up, please follow instructions outlined in the Foreman’s Incident Response Sheet (next page).
• Begin an incident report
• Interview witnesses
• Evaluate and record the circumstances of the incident
• Later, complete the report with the help of the injured worker and Safety
• Assist
Safety in filling out the SAIF 801 form or OCIP form if at Intel
First Aid:
• Apply first aid as appropriate
• Report to Safety

• Return worker to duties
• Report the incident to safety–David Dale or Toby Eidem

• Begin an Incident Report
• Interview witnesses
